Parathyroid hormone (PTH) and l,25-(OH) 2 vitamin D play key roles in the regulation of calcium and phosphate balance. Circulating levels of PTH can change within seconds after an alteration in serum calcium. When the serum calcium falls PTH is rapidly released and acts quickly to promote calcium reabsorption in the distal tubule and the medullary thick ascending limb of the loop of Henley. PTH also stimulates the release of calcium from the rapidly exchangeable pool of bone calcium and a chronic elevation in serum PTH increases renal l,25-(OH) 2 vitamin D production.

While attempting cysto-urethrography and genitography in a nine-year-old female there was extravasation of the contrast medium to the peritoneal cavity during voiding cysto-urethrography (VCU) due to congenital adrenal hyperplasia (CAH). Spillage of peritoneal contrast medium during VCU is extremely rare, even in patients with CAH and a persistent uro-genital sinus. Early fluoroscopic detection is essential, in recognizing the occurrence of spillage and preventing further unnecessary injection of contrast medium into the peritoneal cavity

Amongst clinicians the diagnostic approach differs to patients presenting with isolated third cranial nerve palsy. Some issues remain controversial and the management of this condition is a challenge. In a 38-year-old male presenting with pupil-involving oculomotor nerve palsy (internal dysfunction) the cause was found to be a double posterior communicating artery; a very rare cause of third nerve palsy.

51 year old iraqi male patient diagnosed as ALL started on German multicenter protocol (having a hichman line) developed febrile neutropenia for which he was started on Tazocin and amikacin five days after starting he remains febrile so antifungal was started (later on the culture came positive for Candida albicans) and patient sent for ophthalmologist for evaluation where fundoscpy done and showed the following picture

A 32-year-old man presented to the emergency department complaining of difficulty of swallowing. Four weeks ago he swallowed an unknown quantity of a bleaching substance (sodium hypochlorite) by mistake and was admitted to the hospital for three days. A barium swallow via a naso-oesophageal tube was carried out.

Background: Since some patients with psoriatic arthritis do not respond to typical drug treatments, alternatives are needed. Findings suggest that interleukins 12 and 23 might affect clinical symptoms and pathological joint changes of psoriatic arthritis. Ustekinumab is a human monoclonal antibody that inhibits receptor-binding of these cytokines. We aimed to assess the efficacy and safety of ustekinumab for psoriatic arthritis in this phase II study.

Background: Epidemiological evidence suggests that obesity has effects on bone density. Obese subjects have increased bone density relative to non-obese subjects yet this relationship is not fully understood. However, the influence of these factors has not been well documented in Arabian Gulf women.
